Loss Prevention Question...civil demand error..VERY important..need answer ASAP!!!?

I work Loss Prevention....I apprehended a juvenile shoplifter....I called her mother and she arrived shortly after...the mother signed the release form..and the daughter signed that she admitted theft...the issue is this..I asked the juvenile to sign the civil demand form by accident...and she signed it and I gave her a copy...and the mother doesnt speak english....but she wont get the fine unless I put all the information into the database...(note...the daughter is actually emancepated..married has a child..and lives with her husband)...but Idk what to do...im afraid my boss will fire me for asking a juvenile to sign the civil demand..because the parent is responsible for their "minors" actions..and the parent is supposed to sign the civil demand...I need some advise..should I throw the civil demand paper in the garbage...and say I forgot to ask anyone to sign the civil demand form??? Should I tell my boss I accidentally asked the juviline to sign the form? Or should I just put it in the database.and if my boss asks about it..just say oops Im sorry I made a mistake?? Someone please help me!!! I dont wanna get fired!!!
